Before You Begin
USCIS Form I-9, edition 01/20/25. OMB No. 1615-0047.
Employers must ensure the form instructions are available to employees when completing this form. Employers are liable for failing to comply with the requirements for completing this form. See below and the Instructions.
All employees can choose which acceptable documentation to present for Form I-9. Employers cannot ask employees for documentation to verify information in Section 1, or specify which acceptable documentation employees must present for Section 2 or Supplement B, Reverification and Rehire. Treating employees differently based on their citizenship, immigration status, or national origin may be illegal.
Employees must complete and sign Section 1 of Form I-9 no later than the first day of employment, but not before accepting a job offer.
Where to read the official instructions
Form I-9 comes with a separate instructions document, which the paragraph above says must be available to you while you complete the form.
You can read them, free, at uscis.gov/i-9, together with the Handbook for Employers (M-274) that several entries in the document lists refer to. Ask Apex Health Solutions LLC if you would rather have a printed copy.
What this page is, and what still happens in person
This page collects Section 1 — your part of Form I-9. Section 2 is completed by Apex Health Solutions LLC, in person, within three business days of your first day. Someone there has to physically examine your original documents; that cannot be done online, and nothing you upload or type replaces it.
The next page lists every document that is acceptable. Bring one from List A, or one from List B together with one from List C. The choice of which to bring is entirely yours.
